Delaware Opportunities, Inc. is seeking a full-time Social Care Services Navigator to join our Community Services Department. The ideal candidate is hardworking, reliable, and trustworthy, with the ability to support clients in identifying and addressing unmet health-related social needs.

Responsibilities Include (but are not limited to):

  • Conduct screenings to identify unmet health-related social needs;
  • Assist clients with navigating and accessing services;
  • Determine eligibility for additional Delaware Opportunities programs;
  • Make appropriate internal and external referrals;
  • Develop a Social Care Plan for eligible members;
  • Comply with all applicable Codes of Conduct and Conflict of Interest policies for Delaware Opportunities and the Care Compass Collaborative Social Care Network;
  • Participate in required trainings;
  • Maintain accurate documentation and records;
  • Perform related duties as assigned;

Qualifications:

  • High school diploma or GED and/or 2 years of experience in Human Services;
  • Valid NYS driver’s license;
  • Strong communication and organizational skills;
  • Attention to detail and a non-judgmental approach;
  • Ability to work independently and as part of a team;
  • Ability to meet with clients in office and community settings, including home visits

Schedule & Pay:

  • 35 hours per week
  • Salary: $23.39 - $27.90 per hour depending on experience
  • Monday - Friday
