The Social Care Network (SCN) helps Medicaid members address unmet health-related social needs that impact their stability and well-being. Through screening and care coordination, our team connects individuals and families to supports such as housing assistance, food resources, utility support, transportation, and other community-based services. By addressing these basic needs, SCN services strengthen overall health and quality of life.



SCN services requires enrollment in New York State Medicaid and identification of one or more unmet social needs through screening. Referrals may be made by healthcare providers, community agencies, or through self-referral. Final eligibility is confirmed through assessment, and services are provided based on individual need and program guidelines.
