Delaware Opportunities, Inc. is seeking a full-time Social Care Services Director for our Hamden location. The ideal candidate must be hard working, reliable, and trustworthy. Responsibilities include: Provide oversight to all Social Care Network services and deliverables as well as provides oversight to assigned staff. Provides direct supervision to Agency Advocate, Homeless Advocate(s), and Social Care Services Navigator(s); Leverage the UniteUs platform to receive and manage referrals (within five days); Provide status updates of the service delivery in the UniteUs platform, including when it has been initiated and completed; Obtain necessary approvals from the SCN Lead Entity for the provision of services, when applicable; Attend required internal and external meetings; Report all instances where there is a reasonable cause to suspect that a child is abused or neglected ;Complete all required reporting and data collection activities as assigned; Comply with Code of Conduct and Conflict of Interest policies of both Delaware Opportunities as well as Care Compass Collaborative’s Social Care Network; Participates in required trainings; Perform any similar duties as required or directed.
Skills required are: Good communication and organizational skills. Strong supervisory skills. Non-judgmental. Attention to detail. Ability to work as a team and independently.
Candidate must possess a valid NYS drivers license, Associates Degree in human services, human development, psychology or related area; and/or 2 years experience in human services.
