Seeking energetic, caring, self-starters to support developmentally disabled individuals in the Stamford, Davenport, Sidney, and Walton areas. Create your own flexible schedule based on individual needs, with no weekends or overnights required. All positions allow for either the option of a per diem, part-time or a full time schedule (20+ hours per week) to best suit your availability. High school diploma or GED required. Must have clean NYS driver’s license with no moving violations within last two years. Personal vehicle required; mileage reimbursed.
Description of Responsibilities:
- Meet with prospective families in need of respite and other developmental disabilities services
- After receiving case assignment from Home Care and Developmental Disabilities services Director. Schedule service with family
- Provide service as scheduled and in accordance with guidelines established in individual service and habilitation plans
- Follow all program and agency procedures for operations
- Promote the goals of independence and mutual respect; reinforce developmental of healthy family dynamics
- Refer families to other needed services and advocate on behalf of families and consumers to receive services
- Incorporate parent input, community agencies and Child and Family Development Division resources in the delivery of services after reviewing with the Developmental Disabilities Services Director
- Complete logs on each visit, monitor progress toward established valued outcomes and objectives where appropriate and submit all documentation to Developmental Disabilities Services Director in a timely manner
- Maintain regular contact with Home Care and Developmental Disabilities Services Director and Assistant pertaining to care assignments, schedules of visits, progress made toward valued outcomes, successes and concerns
- Maintain confidentiality concerning all matters related to the provision of developmental disabilities, both current and past
- Become familiar with guidelines for the reporting of incidents of suspected child abuse or maltreatment and act accordingly as a mandated reporter in contacting the State Central Register to report such incidents
- Work with Developmental Disabilities Services Assistant to maintain and improve the quality of care
- Attend meetings as required
- Complete all documentation as required
- Perform any similar duties as required or directed
