Bachelor’s degree, associate’s degree or RN in health or human services field and one year experience working with people with developmental disabilities or one year of services coordination with any population.  To provide services coordination for developmentally disabled individuals and assure access to and coordination of community services primarily in Delaware, Otsego and Chenango Counties.  Responsible for service plans and coordination of community based supports.  35 hours per week.  Salary range $18.46 - $22.02 per hour.  Applications accepted through March 6, 2017 at Delaware Opportunities Inc., 35430 State Highway 10, Hamden, NY 13782.  EOE
